Strategic Alliance
ACAP is pleased to announce a new Strategic Alliance with Health Integrated to develop and offer ACAP-branded, cost-effective, health management programs to ACAP-member Safety Net Health Plans that will help advance their mission to serve the underserved.
This new alliance will to increase the capacity of Safety Net Health Plans to manage complex populations and will provide operational support to ACAP members as they build the capacity to implement the Affordable Care Act. This new relationship will result in ACAP members enhancing their economic and operational capabilities, and sharpening their competitive edge in the marketplace.
Health Integrated will draw on its history of delivering impactful health management solutions for vulnerable populations and tailor its capabilities to the unique needs and expertise of ACAP-member Safety Net Health Plans.
ACAP members will also play a significant role in creating the innovative care management programs that this alliance is intended to foster.
Committees will be established to:
- provide guidance to the alliance;
- develop requirements and timelines to launch ACAP-specific solutions,
- provide direction around best-practice metrics, and
- pool outcomes of ACAP plans running Health Integrated programs.
This new strategic alliance with Health Integrated presents a remarkable opportunity to strengthen ACAP's ability to help its member plans improve health outcomes for those most in need in ACAP-member plans' communities.
