  ACAP WELCOMES NEW STAFF MEMBERS ANDREA MARESCA, TIM MURPHY AND CHRIS SLAGH

Andrea Maresca is returning to ACAP as its Director of Medicaid Policy, replacing Jenny Babcock who recently went to HHS’s new Exchance office. She will be leading ACAP’s Medicaid efforts.

For three years, Andrea worked for National Governors Association (NGA), most recently as Legislative Director for Health Policy, guiding the group’s work on health care reform, Medicaid, the state Children’s Health Insurance program (CHIP), and health information technology. Prior to her work at NGA, Maresca was a Senior Health Policy Associate for the National Association of State Medicaid Directors, and she worked on Medicaid and Medicare issues for the National Association of Community Health Centers (NACHC). She worked at ACAP IN 2004 and 2005. Maresca is also a former aide to Congresswoman Rosa DeLauro (D-CT).

Timothy Murphy joins ACAP as a Program Associate. He will providing policy, research and administrative support for ACAP.

Tim attended American University in Washington, DC receiving a degree in Political Science, with a focus on Public Policy in May of 2010. Previously Tim worked in the Deputy Secretary’s Office at the Department of the Interior, the Employee Benefits Security Administration at the Department of Labor, and interned in Congressman Ed Markey’s Office. While at American, Tim took several courses in public policy, public finances, and public administration.

Christopher Slagh will be working interning with ACAP through the Fall. He will be primarily coordinating ACAP’s upcoming move but also assisting with various program work.

Christopher recently graduated from Georgetown’s School of Foreign Service with a Master of Arts in Security Studies. He also attended Georgetown for his Bachelor of Arts, graduating in May 2009 with a major in English Literature and a minor in Political Science. While attending Georgetown, he participated in two policy-based internships and a variety of campus activities including the DC Schools Project and varsity football.

ACAP HONORS CHAIRMAN WAXMAN WITH CONGRESSIONAL HEALTH LEADER AWARD

ACAP presented its “2010 Congressional Health Leader” award to Rep. Waxman, the Chairman of the House Committee on Energy and Commerce, citing his leadership in expanding health coverage, strengthening public health programs, and preserving the role of safety net health plans.

PRIMARIES OVER, ACAP KEEPING AN EYE ON MIDTERMS

On the heels of a successful fall fly-in, ACAP is still eyeing the shifting sands of the fall election. While Democrats are still on the run, the loss by moderate Delaware Republican Mike Castle to Tea Party candidate Christine O’Donnell and the decision by Senator Lisa Murkowski to run a write-in campaign against the Democrat and the Tea Party candidate that won the Alaska Republican Party’s nomination have definitely changed the dynamics of the Democrat’s efforts to keep the Senate. Over the weekend, representatives from the Administration tried out their new line of attack – suggesting that the Republican Party is purging its independent voices and replacing them with “extremists.” We’ll see if that strategy can move enough of the electorate concerned about a stalled economy, sky-high national debt, and a country moving in the wrong direction.

While Republicans keep telling the voters that they want to “repeal and replace” the health reform law, they are finding that the numbers just won’t add up with a Democratic president in place. So they have decided to take a different approach – defunding the law’s enforcement and programmatic spending. The first shot was taken last week when Republicans supported an amendment to use $11B from the law’s Prevention fund to offset the cost of repealing the law’s new 1099 tax requirements. While the amendment failed, it certainly shows the approach that Republicans will take if they assume control of Congress next year. One bit of good news – changes in Medicaid cannot be made through the appropriations process so the expansion of Medicaid is likely safe for at least two years (but still two years before the changes take effect.) President Obama and some third-party spenders also seem to be getting into the act and going on the offense trying to sell the law. But with few Congressional Democrats embracing the reforms they voted for, it’s questionable how much this new effort will have an impact.

The Medicaid and CHIP Payment and Access Commission, created by CHIPRA and expanded under the Affordable Care Act, has a new website at www.macpac.gov.

ACAP PLANS COME TO DC FOR SUCCESSFUL FALL LEGISLATIVE FLY-IN

ACAP held its 8th Fly-In on September 13 and 14. Over 40 people attended, including 11 ACAP health plan CEOs, from 22 ACAP health plans and representing 13 states.

After lunch and a lobbying briefing, attendees were sent to the Hill on Monday afternoon and all day Tuesday. Together they held nearly 60 meetings to educate Members of Congress and their staff about the importance of Medicaid and safety net health plans.

The fly-in was a great opportunity to thank Congress for the passage of health care reform and to continue building strong relationships with Members and their staff. ACAP’s top legislative priorities included the Medicaid Continuous Quality Act, Actuarial Soundness and Medicare Advantage star quality payments.

HHS STAKEHOLDER CONFERENCE ON EXCHANGES VIDEO

ACAP participated in a one-day conference that HHS hosted on issues regarding implementation of the Exchanges. Stakeholders discussed several aspects of Exchange implementation, from the needs of consumers and employers to general operations, including eligibility and enrollment of participants, plan qualifications and the role of the Exchanges in promoting delivery system reform. Senior Administration officials from HHS, Treasury, Labor and OPM were in attendance to listen to stakeholder feedback.

ACAP CEO Meg Murray was on a panel that discussed the design of the Qualified Health Plans. While ACAP has not yet come up with any formal positions on these topics, Meg did touch on the issues that face the Safety Net Plans as they try to enter the Exchange.

MELANIE BELLA APPOINTED TO COORDINATE MEDICARE, MEDICAID COVERAGE DUAL ELIGIBLES AS NEW DIRECTOR OF THE FEDERAL COORDINATED HEALTH CARE OFFICE

Donald Berwick, M.D., Administrator of CMS, announced on September 13th that Melanie Bella will join the Center for Strategic Planning (CSP) as the Director of CMS’s Federal Coordinated Health Care Office. Most recently, she has been senior vice president for policy and operations at the Center for Health Care Strategies, where she worked on dual eligible issues. Her new office will work to integrate benefits for beneficiaries who are dually eligible for Medicare and Medicaid and improve the coordination of care by improving the health care delivery systems. The office, established by the Patient Protection and Affordable Care Act, will work closely with the Center for Medicare and Center for Medicaid, CHIP and Survey and Certification senior leadership and staff, as well as other federal agencies and states’ Medicaid programs to improve and integrate care for dual eligible beneficiaries across the country.

Melanie served as Medicaid director for the state of Indiana from 2001 through 2005. During her tenure, one of her most notable accomplishments was spearheading the creation of the Indiana Chronic Disease Management Program.

ACAP CHALLENGES MEMBERS TO JOIN TEXT4BABY EFFORT

This is a friendly reminder that at our July Board meeting, ACAP QM Committee challenged all our member plans to consider joining the text4baby initiative as an outreach member. We want to thank and recognize all the ACAP plans that are already participating or have recently joined this effort. Since July, the number of ACAP health plans that are listed as text4baby outreach partners has increased from 12 to 16 (see list below) and at least 3 plans are in the process of becoming outreach members. We would hope to see this number grow even higher.

Text4baby is a free mobile information service designed to promote maternal and child health. An educational program of the National Healthy Mothers, Healthy Babies Coalition (HMHB), text4baby provides pregnant women and new moms with information they need to take care of their health and give their babies the best possible start in life. Women who sign up for the service by texting BABY (or BEBE for Spanish) to a designated number will receive free SMS text messages each week, timed to their due date or baby’s date of birth. Text4baby is made possible through a broad, public-private partnership that includes government, corporations, academic institutions, professional associations, tribal agencies and non-profit organizations. Of key significance is that this service, through support of the major mobile operators via the CTIA Wireless Foundation, is delivered at no charge to the end-user. The only other instance this has happened in the past is for Amber Alerts.

ACAP CONTINUES NETWORKING CALLS ON MEDICAL MANAGEMENT, ICD-10 AND REDUCING DISPARITIES

On September 8th, the CMOs continued the networking call series on Best Practices in Medical Management. The purpose of the call series is allow CMOs an opportunity to discuss effective practices on a variety of topics. The focus of the most recent call was encouraging effective measures to prevent preterm prevention.

The CIOs and COOs continue their networking calls to discuss ICD-10 implementation on September 10th. The focus of the call was on the status of the conducting a gap analysis and the need to develop training programs. ACAP is currently working to develop a training partnership for our plans and will share more information as it develops.
 

Finally, the networking series for Quality Directors on Reducing Health Care Disparities held a session on September 10th. The focus of this call was on specific quality improvement interventions that plans have implemented to combat disparities.

PEGGY OEHLMANN JOINS ACAP ASSOCIATE MEMBER MARYLAND COMMUNITY HEALTH SYSTEM

Peggy Oehlmann is now the Director of Program Management for Maryland Community Health System. In this position, she will be responsible for: the development and implementation of the organization’s strategic plan, creating new policies related to organizational direction and management, enhancing integration of clinical quality improvement into organizational policies, identification and development of new revenue sources, assisting health centers in developing new access points and service capacity, collaborating with community partners and regulatory agencies, and supporting the organizations public policy and advocacy initiatives. ACAP has scheduled a one-day meeting on Accountable Care Organizations (ACOs.) The meeting will take place in Chicago, IL on Wednesday, September 29th from 8:45 am to 3:15 pm. Ingenix Consulting is under contract with ACAP to serve as faculty for this meeting as well as the two scheduled webinars (September 7th and September 22nd at 3 pm ET) and to prepare a paper at the conclusion of the series that will focus on the ACOs and the relationship with safety net health plans. The meeting will focus on both the factors in developing an ACO as well as the issues for a managed care plan to consider in contracting with an ACO. Please note that the content for the 2 webinars is different.

HHS AWARDS $31 MILLION MORE FOR PREVENTION AND WELLNESS PROJECTS

On September 14th, The U.S. Department of Health and Human Services (HHS) announced $31 million for awards to ten communities in eight states and one award to a state health department to support public health efforts to reduce obesity and smoking, increase physical activity and improve nutrition.

The awards funded by the Prevention and Public Health Fund included in the Affordable Care Act are part of the HHS Communities Putting Prevention to Work (CPPW) program, a comprehensive prevention and wellness initiative administered by the Centers for Disease Control and Prevention. The awards included $3.6 million to the Santa Clara County Public Health Department for obesity prevention.

The announcement follows the release in February and March 2010 of more than $491.8 million in Communities Putting Prevention to Work funds to states, U.S territories and communities. Those projects are supporting statewide and community based policy and environmental changes in nutrition, physical activity, tobacco control, expanded tobacco quit lines, and cessation media campaigns.

URAC OPENS 2ND STAGE OF PATIENT CENTERED HEALTH CARE HOME (PCHCH) PROGRAM FOR PUBLIC COMMENT

URAC launched the 45 day public comment period for the second stage of its new PCHCH Education and Evaluation Program on August 31, 2010. The PCHCH Education and Evaluation Program is a complimentary suite of three toolkits to help healthcare practices determine their readiness to become a Patient Centered Health Care Home, as well as steps which can be taken to advance from a basic to advanced a truly patient-centered practice. The PCHCH Evaluation and Education Program is scalable and flexible in design, and can be adopted by individual or multi-site practices, physician groups, health plans, insurers, and pilot programs. The three toolkits can be used individually or in any combination, and include a Healthcare Practice Assessment Toolkit, a Patient Satisfaction/Experience Toolkit, and a Performance Measures Toolkit. This PCHCH Education and Evaluation Program is a voluntary, step-wise, educational approach towards building PCHCH infrastructure and competencies, and thus stands alone as a unique, educational and evaluation product for URAC. End users can decide how to use the toolkits, with the potential for payers and pilots programs to set their own benchmarks for practice qualification for funding incentives and payment bonuses. The multi-stakeholder URAC PCHCH Advisory Group developing the program represents every sector of the health care community, and has provided expert guidance as the program has progressed substantially since the Advisory Group’s kick-off meeting on March 17, 2010. The new comment period focuses on the Patient Survey Toolkit and the Performance Measures Toolkit, which are available for review and comment at http://www.urac.org/publiccomment/.  The deadline for public comment is October 14, 2010.  

AHRQ RELEASES KEY QUESTIONS ON CARE MANAGEMENT

The Agency for Healthcare Research and Quality (AHRQ) Effective Health Care Program is pleased to announce that key questions have been proposed for the following comparative effectiveness review:

  • Comparative Effectiveness of Case Management for Adults with Medical Illness and Complex Care Needs.

These key questions are now open for public comment at http://effectivehealthcare.ahrq.gov/index.cfm/research-available-for-comment/.

CENSUS FINDS DROP IN HEALTH COVERAGE, RISE IN POVERTY

This month the Census Bureau released new figures on the number of Americans without health coverage. According to the new figures, in 2009 the rate of uninsured rose from 15.4 percent to 16.7 percent. This represents 50.7 million people --the highest number on record and the first year that the number of people with health insurance decreased since comparable health statistics have been collected. Between 2008 and 2009, the number of people covered by private health insurance decreased from 201.0 million to 194.5 million, while the number covered by government health insurance climbed from 87.4 million to 93.2 million. The increase in the number of uninsured is largely attributed to the loss of employer-provided health insurance during the recession.

The Census figures also indicated a significant increase in the poverty rate to 14.3 percent, up from 13.2 percent in 2008 — the second statistically significant annual increase in the poverty rate since 2004. There were 43.6 million people in poverty in 2009.

CMS ISSUES PROPOSED RULE ON STATE DEMONSTRATION WAIVERS

The Centers for Medicare and Medicaid Services published a proposed rule in the September 17, 2010 edition of the Federal Register implementing section 10201(i) of the Patient Protection and Affordable Care Act. These provisions establish transparency and public notice procedures for experimental, pilot, and demonstration projects approved under section 1115 of the Social Security Act relating to Medicaid and the Children's Health Insurance Program (CHIP).

In its proposal, CMS plans to increase the degree to which information about Medicaid and CHIP demonstration applications and approved demonstration projects are publicly available and promote greater transparency in the review and approval of demonstrations. States would be required to issue a public notice with a comment period of at least 30 days prior to the State’s submission of a new demonstration application to CMS. States would still be permitted to submit pre-application concept papers and confer with CMS on demonstration projects prior to submitting a complete application. The agency also specifies a list of the elements that must be included in any state application. Once a completed application is submitted, CMS would start a 30-day Federal public comment period. CMS would not issue a decision before 45 days after receiving the completed application. CMS also stated that the agency will regularly update a website with information on the status of all state submissions, including all information submitted.

For an extension of a demonstration waiver, a state must issue public notice of its intent for an extension and receive public comment at least 30 days prior to its submission. The extension application must be submitted at least 12 months in advance of expiration of demonstration.
The agency has asked for comment on several aspects of the proposed rule including the rule’s impact on beneficiaries, providers, and States, the administrative processes, the timeframes described within the rule, the projected impact in sound policymaking at the State and Federal levels, and all aspects of the application process for 1115 demonstrations. Comments will be accepted through November 16, 2010.
