Braided Health supports and serves people who are receiving or are eligible to receive home and community based services. We have developed software and analytics that approach member segmentation more holistically, leveraging functional health as well as physicial and behavioral health needs. Our objective with our analytics and technology is to identify the best possible person centered service plan. Braided then takes on the service coordination role to work with the member and community stakeholders to implement desired and appropriate changes to the service plan. Lastly, Braided has established partnerships with companies that allow us to curate home and community based services that complement personal care services and help better meet an individual’s values and preferences.
