About Cityblock Health’s SDOH Services: 

Cityblock’s model analyzes and assesses social determinants of health to ensure Members receive the appropriate care they need. Cityblock’s Member assessment evaluates across various domains, including but not limited to food, housing, transportation, social/family engagement and financial security. Cityblock employs a Community Health Partner (CHP) who, in addition to managing the Member’s care across the broader healthcare continuum, is responsible for making connections to relevant social services and community based organizations.
