We are committed to enhancing health and birth outcomes through Delfina Care, our virtual maternal health platform. This platform offers personalized care, resource coordination, and culturally sensitive education, utilizing AI-driven technology and human-centered services in English and Spanish. We aim to empower members to make informed healthcare decisions, reducing disparities and boosting engagement. Delfina Care integrates seamlessly with OBGYN workflows, providing insights into patients who may benefit from early interventions to prevent complications. It also streamlines clinical care pathways by enabling home-based monitoring of risk factors such as blood pressure, blood glucose, mood, and weight. We complement primary prenatal care and Delfina’s member-facing mobile app with comprehensive support from Delfina’s bi-lingual virtual care team, which includes OBGYN-supervised:
● Doulas (DONA-International certified),
● Registered dietitians (RD or RDN with relevant state licensure),
● Mental health providers (PsyD, LMHC, or LCSW with relevant state licensure),
● Nurses (RN or NP with relevant state licensure), and
● Lactation consultants (IBCLC or CLC with relevant state licensure).
Our commitment to equity is reflected in our Hypertensive Disorder of Pregnancy (HDP) predictive model, which the Eunice Kennedy Shriver National Institute of Child Health and Human Development (NICHD) recognized for equitable performance. By accurately predicting populations who could benefit from early intervention, our model enables early intervention, particularly for underserved communities, helping to overcome access barriers. Our models are designed to meet the diverse needs of communities, ensuring care is effective, fair, and accessible. Through evidence-based innovation, we strive to reduce health disparities and promote equitable healthcare for all.
Our HDP model’s performance surpasses the American College of Obstetricians and Gynecologists (ACOG) standards. This allows healthcare providers to implement preventive measures more effectively and reduce hypertensive disorders during pregnancy, which disproportionately impact marginalized groups. Additionally, we support addressing social drivers of health (SDOH), such as food access, housing stability, and transportation, through resource navigation support delivered by our team of bilingual virtual Delfina Doulas and partnerships with community organizations, ensuring comprehensive and equitable support for our members.
