We start by validating accurate member data (if a health plan provides a member roster)
reaching out and engaging members and offering immediate and flexible appointments to
ensure members receive the care they need when they need it. We provide scheduled
telehealth visits for preventative and specialty services as well as real time transfers to
providers for an immediate appointment. During telehealth visits providers conduct detailed
assessments to address quality measures along with other clinical needs. Once a visit is
complete, we provide care coordination and referrals, as needed, as well as remote patient
monitoring services to ensure the patient is getting the care they need when they need it.
Finally, we collect and analyze real time health plan data providing clear insights into patient
care. Our dashboards and live client console make it easy for you and your team to track performance and outcomes

connected to 4 social services while they are in our care. Identifying and connecting patients to
social supports allows patients to live a healthy, happy life.
