Founded in 2004, Sagitec specializes in the public sector and healthcare industries. Sagitec’s HealConnect is a HITRUST-certified digital engagement platform designed for Medicaid Managed Care Organizations (MCOs). The platform uses SMS, email and IVR to engage hard to reach members. MCOs use HealConnect to:
a. Follow-up with members after ER visit to ensure adherence with discharge notes. Schedule a call with the care manager. Long term campaigns to address root cause of ER visit and create awareness of other care options.
b. Get members to complete Health Related Social Needs (HRSN) and health risk screenings.
c. Deliver health journey campaigns to members to improve health literacy about their conditions.
d. Send reminders to complete health actions for HEDIS measures like BCS, CCS, AAP.
e. Tailor redetermination messages based on member demographics and engagement.
f. Send appointment reminders on behalf of providers.
g. Deliver gift cards for completion of health actions.

Sagitec’s SDOH Services: 

Engage members for CalAIM related ECM/CSS services in San Diego. The platform is being set up for Community Health Workers (CHW) to connect with members for CalAIM services. The platform is used to enroll members, perform assessments, manage care plans, support CSS delivery and bill for CSS services.
