Waymark is a team-based community care model that partners with health plans and primary care providers to reduce costs and close care gaps.

Waymark trains and employs a team of community health workers, pharmacists, social workers, and care coordinators to assist PCPs caring for patients receiving Medicaid, to help with housing, food, transportation assistance, mental health, and medication access, either at home or in the community.
